Book an Appointment
Submit an Appointment Request (Accurx)
Complete our online form to request an appointment or medical advice. Your request will be reviewed by our clinical team, and we will contact you as appropriate.
Sickness Certificates (Fit Notes)
You must give your employer a doctor's 'fit note' (sometimes called a 'sick note') if you've been ill for more than 7 days in a row and have taken sick leave. This includes non-working days, such as weekends and bank holidays.

Home Visits
Home visits are available only for patients who are housebound or too unwell to attend the surgery.
We encourage patients to attend the practice where possible, as it allows us to provide comprehensive care using our full range of medical equipment. However, we understand this is not always feasible.
To request a home visit:
- Request a home visit online as early in the day as possible
- If you're unable to complete the form yourself, call us at 020 7794 6256 and a member of our team will be happy to assist you in completing it
- In some cases, visits may be conducted by a community nurse or health visitor, depending on your needs.
Change or Cancel an Appointment
Please give us as much notice as possible so we can offer your appointment to someone else.

- Use your NHS account (through the NHS website or NHS App)
- Cancel using the GP online system
- Phone us on 020 7794 6256
- Reply CANCEL to your appointment reminder text message
Out of Hours
Enhanced Access
Cholmley Gardens Medical Centre is open later on Wednesdays for enhanced access appointments.
We are open from 6:30pm to 8pm on Wednesdays. Please ask at reception for enhanced access information.
Life Threatening
Call 999 or go to A&E now if:
- you or someone you know needs immediate help
- you have seriously harmed yourself – for example, by taking a drug overdose
A mental health emergency should be taken as seriously as a medical emergency.
Urgent But Not Life Threatening
Visit an urgent care centre if:
- You have an urgent medical issue requiring on the day attention
Non-urgent
Use NHS 111 if:
- You need help now, but it’s not an emergency
There will be someone to provide you with advice and to direct you to a clinician if it is necessary.
